Game difficulty is certainly a subjective thing, but everyone has played some games that were just HARD. Perhaps it was because the game had broken controls, unfair AI, or was just hard.

My favorite of the hard games I've played is Super Meat Boy. Crazy hard, but rewarding. Especially watching all your fails after you've finally succeeded.

As a kid

Mecha-dragon mecha-wrecked me so bad.

As an adult?

Though it was more about persistence than anything. There was just so much ground to cover in order to unlock everything. Especially with the Afterbirth DLC.

The pixel-precise jumps you had to do in that game were just crazy. This combined with the tough-as-nails enemies meant you had to have a lot of patience and determination to get through the whole thing.

Mega Man 2 is absolutely brutal. It's easily the most difficult Mega Man game I've ever played. I abused save states on the Steam version to get through it. I have no regrets.

And oh my, the Ghouls 'n Ghosts games (plus the Maximo games) are RIDICULOUS! I'm pretty sure I've never beaten a single level in any of those.

I might be a wimp, but I just could never get through Super Mario Bros' World 8-4. I guess I needed to look up and memorize the patterns.

X-COM without cheats is also way up there if we want to get away from the NES.

I would have to say the hardest game I have ever played would be Xenosaga. But as a speedrun, I would have to say Alice: Madness Returns.

Hardest game? The Dark Souls series, especially the latest Dark Souls III. It's a bit faster than the others and I can't get the grip how to evade properly. They always somehow manage to hit me when I try to evade. I'm either just a little bit too slow or roll too close so they always find my hitbox somehow. But that's the nature of that game, the unforgiveness towards the player that makes you want to throw the controller away, but pick it up again to try again.

And then you look at some youtube video's and wonder why you get hit and they don't. aarrrgghhh!!!!
